Welcome to Trinity Health & Wellness (“we,” “our,” or “us”). By accessing our website, scheduling an appointment, purchasing a service, or otherwise engaging with us, you agree to be bound by these Terms of Service. Please read carefully.

1. Medical Disclaimer

  • Trinity Health & Wellness provides direct-pay functional medicine services.

  • We do not accept insurance, submit claims, or provide superbills unless otherwise specified.

  • Services are not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911 immediately.

  • While we provide functional and integrative care, results may vary depending on individual factors. No outcome is guaranteed.

2. Eligibility

  • You must be at least 18 years old to become a client.

  • If under 18, a parent or legal guardian must provide consent and accompany you.

3. Payments and Refunds

  • All services are cash-pay / direct-pay only.

  • Payment is due at the time of booking or at the time of service.

  • Consultation fees (e.g., “Clarity Consults”) are non-refundable but may be applied toward a program if you choose to move forward.

  • Program and treatment fees are non-refundable once services have begun.

4. Appointment Policies

  • We require at least 24 hours’ notice for cancellations or rescheduling.

  • Missed appointments or late cancellations may result in a cancellation fee.

  • We reserve the right to terminate services if a client repeatedly misses appointments or violates these Terms.

5. Prescriptions and Supplements

  • Prescriptions, peptides, hormones, or supplements may be offered when medically appropriate.

  • Pharmacy costs and supplement purchases are separate from service fees.

  • Clients are responsible for verifying any insurance coverage for outside prescriptions or labs.

6. Technology and Communication

  • By engaging with us, you consent to receive electronic communications (emails, text reminders, patient portal messages).

  • While we use secure systems, electronic communication may carry some risk of unauthorized access.

7. Website and Content Use

  • All content on our website (text, graphics, branding, etc.) is owned by Trinity Health & Wellness and may not be copied, distributed, or used without permission.

  • Any health information provided on our site or social media is for educational purposes only and should not be taken as individualized medical advice.

8. Limitation of Liability

  • To the fullest extent permitted by law, Trinity Health & Wellness and its providers are not liable for indirect, incidental, or consequential damages arising from your use of our services.

  • Our liability is limited to the amount paid for the specific service provided.

9. Governing Law

These Terms are governed by the laws of the State of Florida. Any disputes shall be resolved in the courts located in Pasco County, Florida.

10. Changes to Terms

We may update these Terms of Service at any time. Updates will be effective immediately upon posting on our website.
