1. Practice Structure and Scope

Trinity Health & Wellness is a direct-pay functional medicine and primary care practice owned and operated by Leanne Zahn, APRN, FNP-BC, an autonomous nurse practitioner licensed in the state of Florida. We offer root-cause clinical care through The Root Reset™, a structured, six-month private wellness program. We do not provide acute emergency care, crisis intervention, or 24/7 access.

By becoming a client, you acknowledge that your care will be delivered outside of the traditional insurance-based system and will focus on prevention, optimization, and functional medicine–based interventions.

2. No Insurance Billing or Superbills

Trinity Health & Wellness does not accept insurance, Medicare, or Medicaid. We do not submit claims or provide superbills for reimbursement.

All lab tests, supplements, peptides, prescriptions, and specialty services are paid for directly by the client and are not included in your program fee unless explicitly stated.

3. Consultation and Program Fees

  • Clarity Consultations are $150 and are non-refundable. If you proceed with enrollment into The Root Reset™, the full amount will be credited toward your program cost.

  • The Root Reset™ Program is $4,500 paid in full or $875/month for 6 months. This includes all scheduled clinical consultations, portal access, lab interpretation, and protocol design.

  • Payment plans must be completed in full regardless of pace of progress or client withdrawal from care.

4. Refunds and Cancellations

All program payments are final. Due to the nature of the services provided, no refunds will be issued once a program has begun.

If you need to cancel or reschedule a session, we require at least 48 hours’ notice. Missed or late-cancelled appointments may be forfeited or billed at the provider’s discretion.

5. Client Expectations

Clients are expected to:

  • Complete intake forms and lab orders in a timely manner

  • Communicate respectfully and professionally

  • Adhere to the clinical recommendations provided unless otherwise discussed

  • Understand that healing is not guaranteed and is influenced by lifestyle, compliance, and individual biochemistry

We reserve the right to discontinue care if a client demonstrates non-compliance, inappropriate behavior, or misalignment with the scope and structure of our practice.

6. Medical Records and Communication

We maintain HIPAA-compliant electronic records and offer secure messaging through our patient portal. Routine questions are answered within 48 business hours. Trinity Health & Wellness is not a 24/7 or urgent care provider.

For all urgent medical concerns, contact your primary care physician, call 911, or visit an emergency room.

7. Lab Testing and External Providers

All lab testing is ordered through private cash-pay channels. We do not submit orders through insurance-based labs. Clients are responsible for payment directly to lab vendors. Results will be reviewed only within the context of the program.

We do not coordinate care with outside providers. Any labs or treatments from third-party clinics will not be reviewed or interpreted outside of scheduled program sessions.

8. Program Access and Termination

Program access begins upon signed agreement and payment. If client communication ceases for more than 30 days without notice, services may be paused or terminated at the discretion of the provider.

9. Intellectual Property

All protocols, documents, recommendations, and resources shared through the program remain the intellectual property of Trinity Health & Wellness and may not be copied, distributed, or shared outside of the client relationship.

10. Legal Acknowledgement

By becoming a client, you agree to these Terms of Service and acknowledge that Trinity Health & Wellness provides wellness care under the laws of the state of Florida by a licensed autonomous nurse practitioner. This agreement does not establish a primary care relationship for emergency or urgent services.
